Patient Registration Procedure

It is the policy of Sunayana eye hospital to register all the patients coming for the first time, Before seen by the consultant Doctors.

A unique UHID NO is allotted for the new registrations and the card will be issued to the patients.

Demographic data like Name, Age, Gender, Contact number, Address will be used for registration

Step Three Sunayana Eye Hospital Out Patient

Comprehensive Eye Checkup By Our Specialists Which Includes:

Visual acuity testing.
Refraction.
Slit lamp examination of the anterior segment.
Tonometry or IOP checkup for people above 40 years of age.
Dilatation test for fundoscopy or retinal examination.

In the above examination, if any problem is noticed, then they will be referred to the respective department for further investigation.
